Job description :

  • Setting : Skilled Nursing Facility, Sub-acute and long-term care patients.
  • Schedule : Minimum of (2) days on site per week. Telehealth support is available on-site for virtual interactions.
  • Must be licensed in NY
  • Experienced in SNF preferred, not required.
