Care Management is a collaborative practice model including the patients, nurses, social workers, physicians, other practitioners, caregivers and the community. Required Qualifications :

• Graduate of an accredited school of nursing.
• Upon hire : California Registered Nurse License
• 3 years clinical experience in an HMO, medical group, affiliated model, hospital or medical office / clinic setting.
