Job Title : Admissions Nurse – Hospice / Palliative / Oncology

Company : Superior Hospice (part of the Home Health Care, Inc. family)

Compensation :   $200 - $250 Per Admission

Benefits : Full Benefits Package (Full-Time status only)

Location : Greater Twin Cities Metro Area – 21‑county service area

Schedule : Full-Time or Part-Time | Includes 1 on-call shift per week (backup) and every 4th–7th weekend

Position Summary :

We are seeking a skilled and compassionate Admissions Nurse (RN) to serve as the face of our hospice team across communities in the Greater Twin Cities. This is a road warrior position perfect for nurses with prior hospice, oncology, or palliative care experience , either directly in hospice or in acute-care settings with patients nearing end-of-life. This role is integral to our growing team, with room for personal and professional fulfillment.

Key Responsibilities :

• Travel to patient residences and senior communities to complete hospice admissions.
• Assess clinical eligibility and initiate services with warmth, clarity, and professionalism.
• Educate patients and families on hospice philosophy, benefits, and next steps.
• Collaborate with physicians, intake coordinators, and interdisciplinary teams.
• Document all clinical findings and care coordination accurately and promptly.
