Staff Nurse RN (1.0 FTE) - PMR & TBI

SUMMARY

Hennepin Healthcare is seeking a full-time Staff Nurse (RN) to join our Physical Medicine & Rehabilitation (PMTR) and Traumatic Brain Injury (TBI) programs . This daytime position (Monday–Friday, 8 : 00am–4 : 30pm) plays a vital role in supporting the TBI Outpatient Program and the PM&R clinic . The Staff Nurse will focus on care coordination , assisting patients as they transition from hospital or Knapp Rehabilitation to outpatient care, ensuring continuity of services and comprehensive support throughout recovery. In addition, the nurse will provide clinical support for injections and procedures as needed , contributing to safe and effective patient care.

RESPONSIBILITIES

Within the context of the Hennepin Healthcare System Relationship-Based Care practice model, and under general supervision, the registered nurse provides and coordinates nursing care by :

• Protecting, promoting wellness and optimizing health
• Triaging patients by assessing severity of symptoms and referral to appropriate setting or level of care
• Alleviating suffering through the treatment of the human response
• Partnering with the patient, their family and significant others
• Documenting interventions and an individualized plan of care
• Educating patient on their disease process
• Assuring continuity of care
• Educating patient and family in preventative care which promotes health and well being
