Job Summary

The Residential Program Specialist is responsible for providing direct care coordination, case management, and program oversight in a residential rehabilitation setting, in compliance with Pennsylvania's Chapter 6400 regulations under the Office of Developmental Programs (ODP). This role promotes a safe, structured environment that supports independence and personal growth for individuals with developmental disabilities. The Specialist ensures services and care align with ODP standards, while advocating for the well-being and autonomy of individuals. Key responsibilities include completing individual assessments, coordinating support services, conducting HRST (Health Risk Screening Tool) ratings, and acting as a liaison between families, external agencies, and Lehigh Human Support Services (LHSS).

Duties

As a Program Specialist at LHSS, you will play a vital role in ensuring regulatory compliance and supporting person-centered care. Responsibilities include :

Completing all required documentation such as quarterly reports, annual assessments, and incident reports, while maintaining accurate and up-to-date records for individuals and support coordinators.

Conducting and submitting HRST (Health Risk Screening Tool) ratings for all individuals as required, to ensure proactive health monitoring and risk management.

Acting as a liaison between interdisciplinary team members, external agencies, and families to meet individual needs and regulatory expectations.

Initiating and maintaining regular contact with Supports Coordinators to coordinate and schedule ISP meetings, annual reviews, and monthly monitoring sessions.

Collaborating on the development, review, and updates of Individual Support Plans (ISPs), ensuring timely reporting of any discrepancies and contributing accurate, relevant information.

Reviewing the ISP with both individuals and direct support staff to ensure accurate implementation of goals and program alignment.

Providing informal counseling and support to individuals related to workplace and social issues, promoting self-advocacy and problem-solving skills.

Delivering or facilitating training sessions to individuals and staff on topics such as workplace behavior, career paths, conflict resolution, and self-advocacy.

Participating in emergency response and crisis support protocols, as required by program standards.

Supervising or planning individual and group activities that support ISP goals and community integration.

Attending and actively participating in individuals’ medical appointments and ensuring follow-up coordination as needed.

Assisting with or providing coverage for other program staff during absences or staffing shortages.

Performing other duties as assigned by the Quality Manager.

Additional Requirements :

A valid driver’s license is required for transporting individuals as needed.

Qualifications

One of the following is required to qualify :

Master’s degree from an accredited college or university and 1 year of work experience working directly with individuals with intellectual disabilities and / or autism.

Bachelor’s degree from an accredited college or university and 2 years of work experience working directly with individuals with intellectual disabilities and / or autism.

Associate degree or 60 college credits from an accredited college or university and 4 years of work experience working directly with individuals with intellectual disabilities and / or autism.

Skills :

As a Program Specialist, you will use your strong organizational and project management skills to develop and implement program strategies. You will communicate effectively with stakeholders and team members to ensure program success. Your ability to analyze data and evaluate program effectiveness will be critical in identifying areas for improvement. You will also use your grant writing and management skills to secure funding and manage program budgets. Finally, your knowledge of program evaluation and quality improvement methodologies will be essential in ensuring the delivery of high-quality care and services to our clients and patients.
