• Direct the medical care and treatment of patients and have primary medical responsibility in overseeing and supporting the care rendered by other members of the interdisciplinary care team
• Monday to Friday 8-5pm schedule (40 Hour work week)
• Will be based out of clinic but may need to go into the field and go to patients homes, but there are home aides that do this
• Collaborates with site NP community-based primary care providers on the shared population as well as social workers and dieticians
• Will have own patient panel as Primary Care Provider and participates in direct medical care in the Diagnostic and Treatment Center providing med management, patient assessments, and examinations
• Will be overseeing clinical staff (NPs and MAs)
• Participates in developing preventive health screening, immunization programs, professional community outreach, quality assurance, etc.
