About this Job : We are seeking a dedicated and passionate Homecare Physical Therapist to provide exceptional patient care through evaluation and treatment in alignment with agency standards and regulatory requirements in Maryland, the District of Columbia, or Virginia.
Key Responsibilities :
- Assess and document patient and family responses to treatment, evaluating progress toward goals and adapting care plans accordingly.
- Create personalized teaching plans for patients and their families, ensuring effective communication at all stages of care.
- Uphold department policies and quality standards, participating in mandatory training and contributing to departmental goals.
- Develop and implement effective treatment plans in collaboration with the patient, their family, the physician, and the healthcare team, making adjustments as necessary.
- Conduct comprehensive evaluations, including interviews and specialized assessments to guide treatment interventions.
- Organize and prioritize patient assignments, seeking support for complex cases as needed to maintain high standards of care.
- Actively participate in case conferences, involving all relevant healthcare professionals and focusing on patient and family goal-setting.
- Foster strong working relationships with multidisciplinary teams and participate in quality improvement initiatives.
- Perform detailed assessments to identify rehabilitation needs, recommending adaptive equipment and home modifications to enhance patient safety and functionality.
- Execute a variety of therapeutic procedures, including exercises, functional mobility training, and the use of modalities such as heat / cold application and ultrasound.
Minimum Qualifications :
Education : Bachelor’s degree in Physical Therapy from an accredited institution.Experience : 1-2 years of clinical experience is required.Licenses and Certifications : Must possess a valid Physical Therapist License for practice in the District of Columbia, Maryland, or Virginia upon hire. A valid driver’s license and dependable vehicle are also necessary. CPR certification for healthcare providers must be obtained within 90 days of hire.Skills and Abilities :
Strong clinical and organizational skills.Exceptional verbal and written communication abilities.Familiarity with home care equipment and reimbursement regulations.Basic computer proficiency and smartphone experience.This position offers a competitive salary range of USD $74,214.00 - USD $134,596.00 per year.
