POSITION : Hospice Clinical Liaison - Full-Time (Monday - Friday, 8 : 30am - 5 : 00pm)
POSITION SUMMARY :
The Hospice Clinical Liaison serves as a vital link between Hospice of the North Coast (HNC) and the community, blending clinical expertise with outreach and relationship-building skills. This role is ideal for a licensed RN, LVN, LCSW, or MSW who brings both healthcare experience and a passion for connecting with others. The Clinical Liaison is responsible for increasing hospice admissions through strategic referral development, education, and community engagement. With a strong understanding of hospice and palliative care, the Liaison collaborates closely with HNC's interdisciplinary team to ensure timely, compassionate care while serving as a trusted clinical resource to patients, families, and providers.
KEY RESPONSIBILITIES :
- Drive admission growth and meet strategic referral targets (10% growth each quarter after ramp-up period)
- Educate hospitals, skilled nursing facilities, physician practices, and community partners on hospice services and eligibility
- Develop and maintain strong relationships with referral sources
- Act as a clinical resource and bridge between patients, families, and referring providers
- Collaborate with HNC's clinical team to ensure excellent care coordination and timely response
- Represent HNC at community and professional events with confidence, empathy, and professionalism
- Effectively deliver presentations and engage in public speaking about hospice care, services, and eligibility
QUALIFICATIONS :
Licensed RN, LVN, LCSW, or MSW (required)2+ years of healthcare sales, education, outreach, or community liaison experienceDeep understanding of clinical care and hospice philosophyExcellent communication, public speaking, and relationship-building skillsSelf-starter who thrives on purpose-driven work and is comfortable with autonomyValid driver's license, reliable transportation, and current auto insuranceBilingual English / Spanish a plusCORE COMPETENCIES :
