Outreach : NoTelehealth : NoNumber of Physicians : 6Number of Advanced Practice Providers : 2Number of Exam rooms : 13 OR roomsMedical Imaging : NoLab amp Pharmacy : NoClinic Provider Daily Volume : 30-50 cases for whole department per dayEMR : EPICServices offered in the practice : General Anesthesia with OB includes TOLAC and healthy peds. No hearts or neurosurgery.Conscious Sedation is required. Bill Rates Needed for Submission : Malpractice must be included in the ratesOrientation 252 rate capHourly 295 rate capHoliday Hourly Overtime 442.50 rate capHoliday OvertimeNight Call 400 rate capHoliday Night Call24 Hour On Call 1350 rate capHoliday 24 Hour On CallCall Back 400 rate capHoliday Call BackConversion fee 15k rate capTravel Reimbursement : Flights - 750 / round trip cap to include all taxes and fees.
