LTSS Service Care Manager - J01031

Location : Remote and Field visits to members Homes Required, Source from Mission, Edinburg, McAllen, Elsa

Shift : 8am-5pm with 1 hr lunch break . No Overtime Required

Duration : 6 months

Job Description :

• Assists in developing, assessing, and coordinating holistic care management activities to enable quality, cost-effective healthcare outcomes. May develop or assist with developing personalized service care plans / service plans for long-term care members and educates members and their families / caregivers on services and benefits available to meet member needs.
• Assist in telehealth biannual assessments, in person as needed if in the area.
• Complete assigned tasks and document findings.
• Coordination with multi-disciplinary teams.
• Follow up calls to members.
• Any other projects that may arise.
• Must be able to meet strict turn-around times with documentation for assessments assigned.

Must haves : Microsoft Word, Excel experience, Computer literate

Nice to haves : At lease 2 years experience at LVN, Home Health experience, Case management experience

Disqualifiers : No vehicle, no internet
