Nurse Practitioner

Job Description

Nurse Practitioners work in our outpatient clinics as part of a collaborative care team that includes physicians, PAs / NPs, medical assistants, scribes, and nursing support. With access to top specialists and hospitals, providers can focus on in-clinic primary care, care coordination, and population health.

Core Responsibilities :

• Deliver high-quality primary care and manage routine and acute visits.
• Perform annual wellness exams and health assessments with thorough, accurate documentation.
• Coordinate care with specialists, facilities, and community agencies.
• Support population health efforts with the Care Team.
• Assist with phone triage, patient outreach, and health education.
• Participate in clinic and community promotional activities.
• Conduct home visits when needed and perform other assigned duties.
