NATURE OF WORK : To provide nursing services according to physician's orders. This includes taking and monitoring vitals, performing blood draws, providing support for clinical and medical staff, and receiving and directing clients to their appointments.
WORK ENVIRONMENT : In-Doors 95% of the time; outdoors and transportation areas 5% of the time
WORK PACE : Pace determined by the need of client(s).
ESSENTIAL DUTIES :
- Engages clients in achieving their daily goals and objectives through active treatment.
- Completes all job duties as assigned within the specified timeframe or shift as scheduled. Examples of such standard duties include but are not limited to completing shower schedules, dressing and changing clients appropriately, preparing and following meals plans and schedules, notifying the supervisor of items needed by the clients, completing shift to shift reports, following cleaning schedules, accompanying clients to doctor appointments as schedule, completing van runs, reporting incidents to supervisors, completing van logs and following van procedures, completing work orders, etc.
- Provides nursing services according to physician orders. This includes following approved standard nursing practices and guidelines when administering or directing to have administered all medications to clients. This also includes proper documentation and timeliness of the administration of medications as well as observance of the client.
- Provide basic support to clients to ensure they understand the services being offered.
- Provides basic support and consumer education regarding the benefits and potential side effects of prescribed medications and other health related subjects to clients.
- Monitor clients to ensure their needs are meet per their treatment plan.
- Perform daily monitoring services such as checking supplies, tracking inventory and stocking same, having adequate medication samples available, etc.
- Work with other members of the Medical Team to coordinate services.
- Answers / returns phone calls to the Nursing / Rx / Medical Line to assist individuals in need assistance or RX refills.
- Work with other agencies, hospitals, providers, etc. to ensure the referral process is offered.
- Assure that the safety, health and well being of consumers are always maintained on site.
- Monitor, maintain and use appropriately, all equipment.
- Support and help in the training of new employees.
- Attends in-service training's and mandated meetings as assigned.
- Complies with Valley's Policies and Procedures.
- Complies with Licensure Code of Ethics.
- Keeps up to date with knowledge or field and maintains required CEU's for Licensure.
- Adheres to professional code of ethics in performing nursing services including Valley's Standards of Conduct and Compliance.
- Uses clear communication skills, including "people first" language with clients and other professionals.
- OTHER DUTIES AS ASSIGNED.
Qualifications
MINIMUM QUALIFICATIONS :
Ability to perform Essential Duties as outlined below.High School Diploma or equivalent.Current, valid W.V. Licensed Practical Nurse License.Valid Photo ID or Drivers LicenseAbility to comply with Consumers Rights.Ability to comply with Valleys and Division departmental safety procedures.Ability to read, write and speak the English language.PREFERED QUALIFICATIONS :
Experience in Crisis work and / or Substance Use Detox treatmentExperience in Mental health field and / or Substance Use field.Knowledge of diverse disability populations.Knowledge of community resources for behavioral health issues.
