Role Purpose

Provides skilled treatments to patients as prescribed by the physician and directed by a licensed physical therapist as specified in their health care service setting.

What You'll Do :

• Implement Treatment Plans : Provide skilled therapy services to patients in their homes, implementing individualized treatment plans developed by the Physical Therapist.
• Deliver Patient Care : Focus on therapeutic exercises, mobility training, and assistive device instruction to support patient recovery.
• Educate and Encourage : Instruct patients and families on home exercise programs and reinforce treatment procedures.
• Monitor and Report Progress : Observe and document patient progress, reporting changes to the Physical Therapist.
• Collaborate with Therapists : Work closely with Physical Therapists and other members of the healthcare team to ensure coordinated and effective care.
• Promote a Safe Environment : Assist in assessing home environments and report any safety concerns to the Physical Therapist.
• Maintain Documentation : Ensure accurate and timely documentation within the Epic EMR system.
• Contribute to Care : Assists with guidance for clinical fieldwork students under the supervision of the physical therapist. Reports on interventions and patient progress during assigned meetings and contributes to the screening and evaluation process.
