Nurse Practitioner - Rutland / Castleton Express Care (Per Diem)

Community Health Center sites are certified Level 111 Medical Homes, with a highly developed and supportive panel management and case management system in place. Community Health also has a robust integrated behavioral health program employing Psychiatrists, Behavioral Health Nurse Practitioners and Licensed Clinical Social Workers embedded within its primary care sites. We have a fully integrated electronic medical record and practice management system (Medent) with robust interfaces with our local hospital.

POSITION SUMMARY :

Community Health is seeking a Nurse Practitioner to work at our Castleton Express clinic. Candidates are expected to be knowledgeable and skilled in the delivery of express care. Responsibilities include, but are not limited to :

• Obtains complete medical history and physical examination on patients.
• Reviews medical records.
• Interprets and integrates data to determine appropriate diagnostic and therapeutic procedures.
• Establishes presumptive diagnosis.
• Orders appropriate laboratory and diagnostic procedures.
• Develops a therapeutic plan for patient.
• Sutures wounds.
• Prepares written prescription orders for drugs and controlled substances.
• Instructs patients and family regarding medications and treatment instructions.
• Provides recommendations regarding health and illness prevention.
• Recommend community resource to meet patient / family needs.
• Performs independent evaluations and treatment procedures in emergency situations.
• Provides monitoring and continuity of care between physician visits.
