Mileage can be reimbursed. This position provides case management and direct patient care within a team environment. Administers comprehensive physical therapy modalities in the home setting. The qualified therapist assists the physician in evaluating the level of function. Helps develop the plan of care and revises it as necessary. Prepares clinical and progress notes. Provides advice and consultation with the family and other personnel. Participates in in-service programs. Ensures Care Delivery Services are consistent with the agency vision, mission, values, and strategic goals. Promotes effective working relationships and works effectively as part of the team. Utilizes resources, supplies, and time efficiently to contain costs and increase the value of all services to the community. Develops and maintains a customer-oriented environment responsive to the needs of all customers. Direct Patient Care Duties include developing and revising plans of treatment and discharge plans. Collaborates with the patient, family, physician, care delivery team, and other parties. Initiates appropriate rehabilitative programs. Provides an environment that promotes respect for patients, their privacy, confidentiality, and property. Implements home therapy programs including gait training, transfers, and activities of daily living. Periodically evaluates the patient's progress and makes necessary adjustments to the physical therapy program. Consults with and educates patients, families, and other team members regarding disease processes and self-care techniques. Directs, coordinates, evaluates, and supervises the quality of patient care services provided. Interacts with patients, physicians, and referral sources in a manner conducive to positive relationships. Ensures problem resolution at the point of contact. Participates in regularly scheduled patient conferences for discussing patient care and issues. Performs other duties to support the efficient operation of the team. Assures completion of all appropriate clinical records for compliance. Refers to appropriate community agencies and maintains knowledge of available services. Demonstrates flexibility, enthusiasm, and willingness to cooperate while working with others. Ensures that the patient's needs are met and responds to customer needs effectively. Ensures practices, standards, and behaviors meet performance competencies. Participates in the attainment of operational and financial objectives.
