Job descriptionExperience : At least 1,000 hours of experience with geriatric or chronically ill individuals is preferred. Exceptions may be made for CNAs, HHAs, or those focused on companionship and housekeeping. Certifications : CNA or HHA certification is a plus but not required. Health and Safety : Proof of current TB testing, ability to assist with client transfers, and compliance with infection control practices. Transportation : Reliable transportation, a valid driver’s license, and automobile insurance are necessary for client transport. Background Check : Must pass a criminal background check (both national and state-required). Communication : Strong communication skills are essential for interacting with clients, family members, and team members. Implement Care Plans : Follow personalized care plans to ensure clients receive the care and support they need. Provide Support : Assist clients with companionship, meal preparation, light housekeeping, transportation, and personal care (bathing, dressing, toileting, feeding). Advocate for Clients : Report any changes in client conditions to the Care Manager and advocate for their needs. Follow Safety Protocols : Adhere to safety procedures and infection control practices, and notify the office if additional supplies are needed. Accurate Documentation : Log hours, tasks, and client status accurately using the ClearCare Telephony system. Professionalism : Maintain a professional appearance and follow company policies and procedures. Ability to assist with client transfers using proper techniques and equipment. Reliable transportation to clients’ homes.
