Family Practice / Primary Care Nurse Practitioner

• Full time schedule in patient homes and at home : Monday through Friday 8 : 30 - 5 : 00 pm
• Home care or hospice experience preferred
• Chronic disease management experience preferred
• Must be willing to travel within the designated market, up to 1 hour from home
• Must be willing to visit patients in their homes 4 days / week, 1 day remote

Pay flexible

For this location, there will be opportunity for travel. Typically 1x a month and could involve driving 2 hours (maybe more) away, staying overnight, or spending up to a week conducting visits.
