Description :
The Clinical Nurse is a Registered Professional Nurse who provides safe and competent quality care to a designated group of patients and their significant others.
Delivers patient care by incorporating the tenets of a professional practice model through the application of the nursing process based on theory, research, evidence-based practice, and approved organizational nursing standards.
Assesses and evaluates patient care needs while applying critical thinking skills in patient care management.
Integrates relevant assessment and intervention skills in the delivery of nursing care.
Collaborates with the interdisciplinary healthcare team in the development, implementation, and evaluation of the plan of care.
Prioritizes all aspects of patient care including teaching, rounding, coaching, and planning after hospital care, and delegating to others as appropriate.
Educates patients and caregivers about their plan of care, transition of care, promotion of health, and prevention of disease, while anticipating their needs and readiness to learn.
Communicates effectively and professionally with patients, families, and all members of the interdisciplinary patient care team.
Manages assignments within the Care Delivery Model and demonstrates effective allocation of resources.
Role models the principles of patient-centered care through various patient engagement actions.
Considers the patient's values, preferences, cultural diversity, expressed needs, and knowledge in all aspects of care.
Establishes and maintains therapeutic relationships with patients and families.
Uses evidence-based practices to increase understanding of patients' perceptions of care.
Initiates service recovery for patients and family members who have concerns, escalating as needed.
Demonstrates caring, respect, compassion, empathy, and active listening in all interactions.
Supports staff use of evidence-based practices to enhance understanding of patients' experiences.
Practices safe, efficient, and patient-centered care while identifying and addressing barriers.
Demonstrates clinical skills and knowledge critical to quality and safety in the patient population served.
Implements process improvement strategies based on institutional and evidence-based practices.
Engages in peer and institutional review processes both formally and informally.
Collaborates with interdisciplinary teams to create quality improvement projects and evidence-based practice activities.
Contributes to quality and safety practices and compliance measures.
Implements principles of high reliability to mitigate high-risk behaviors and ensure a safe environment for patients.
Escalates potential safety hazards or gaps following the established protocol.
Documents nursing practice accurately and thoroughly to support quality improvement initiatives.
Performs charge nurse duties as assigned and coordinates with management to ensure adequate resources.
Escalates resource deficits as necessary and demonstrates patient-focused approaches to care.
Documents in compliance with all requirements and standards of care.
Facilitates patient throughput effectively during assigned shifts.
Contributes to the environment of care supporting clinical colleagues and patients.
Advances clinical competence in nursing practice toward expertise.
Projects a professional image and communicates with care, respect, and empathy.
Incorporates ethical principles into decision-making for patients and families.
Fosters a culture of inquiry, lifelong learning, and innovation among peers.
