Responsibilities :

• Oversee the social services department, including case management, counseling, and recreational activities
• Develop and implement policies and procedures to ensure compliance with regulatory requirements and company standards
• Collaborate with the medical team to identify and address resident needs and concerns
• Manage and supervise a team of social workers and activity professionals
• Conduct resident assessments and develop individualized care plans
• Coordinate with families and healthcare providers to ensure seamless transitions and care coordination
