Travel Physical Therapist (PT) - $2,468 per week in Bedford, NH

Therapist - Physical Therapy Profession : Therapy / Rehabilitation Specialty : PT Home Health Duration : 13 Weeks Shift : Day Hours per Shift : 8 Experience : Minimum 1 full year Adult Home Health Experience License : Must have license in hand Certifications : CPR, BLS Must-Have : Recent Home Health experience Description : Home visits required for patient care. Periodic office meetings will be scheduled. Must have reliable transportation and a current driver's license. Agency-required liability insurance is necessary. Average number of patients per day is 5-6. Each type of visit and activity holds a different point value. Contractor will be assigned between 5-10 patients per day. This equates to roughly 6-8 points daily. In the event of a low patient census, please discuss financial needs with the manager. Traveling up to 75 miles from the home clinic is required. Mileage is reimbursed at the IRS standard rate. The commute to and from the first and last patient is not billable. Local candidates are accepted. Candidates must understand that all documentation must be completed within 24 hours of the appointment. Delayed and incomplete documentation will impact billing and follow-up patient care. There are no guaranteed hours with this contract. Contractor must meet weekly productivity expectations of 30 points. Declining patients may result in contract cancellation. Candidates must be proficient in HomeCare HomeBase EMR. Travelers must be open to on-call rotations. The contractor will cover specified territories.
