Family Practice / Primary Care Nurse Practitioner

• Hybrid, full time schedule in patient homes and virtually : Monday through Friday 8 : 30 - 5 : 00 pm
• Home care or hospice experience preferred
• Chronic disease management experience preferred
• Must be willing to travel up to one hour from home in the Denver area (can live in Denver, Boulder, Aurora, or Centennial)
• Must be open to traveling to Utah and occasionally New Mexico every 6-8 weeks for 2-3 days at a time
