Job Title : Travel Pharmacist Facility : UC Davis Health Rehabilitation Hospital, Sacramento, CA Shift : Full-time, travel assignment

Overview : UC Davis Health Rehabilitation Hospital is seeking a Travel Pharmacist to join the inpatient rehabilitation care team. The ideal candidate will provide comprehensive pharmaceutical services to ensure safe, effective, and evidence-based medication use for patients recovering from acute illness, injury, or surgery.

Responsibilities :

  • Review and verify physician medication orders for accuracy, appropriateness, and safety.
  • Prepare, dispense, and monitor medications for inpatients in accordance with hospital policies and regulatory standards.
  • Provide drug information and therapeutic recommendations to physicians, nurses, and other healthcare professionals.
  • Monitor patient medication therapy, adjusting as necessary to optimize outcomes.
  • Educate patients and families on medication use, side effects, and adherence strategies.
  • Maintain accurate records and documentation in electronic health systems.
  • Ensure compliance with infection control, safety, and pharmacy regulations.

    • Qualifications :

  • Current Pharmacist license in the state of California.
  • Doctor of Pharmacy (PharmD) degree from an accredited program required.
  • Minimum of 2 years of recent inpatient or rehabilitation hospital pharmacy experience preferred.
  • Strong knowledge of pharmacotherapy, drug interactions, and dosing in complex patient populations.
  • Excellent communication, collaboration, and organizational skills.
  • Ability to work independently in a fast-paced clinical environment.
