About the Opportunity

We're seeking an experienced Nurse Practitioner or Physician Assistant (minimum 2 years) to join our collaborative, physician-led team in St. Louis, Missouri . This role focuses on delivering high-quality, compassionate care to residents in skilled nursing and assisted living facilities.

Responsibilities

Manage acute and chronic conditions for residents onsite

Prescribe treatments, order tests, and educate patients / families

Collaborate closely with physicians and clinical teams

Document care in a provider-friendly EHR

Contribute to facility performance improvement and staff education

Participate in shared call coverage

Stay current with clinical advancements and best practices

Qualifications

Active NP or PA license in Missouri and / or Illinois

National board certification and MSN preferred

2+ years of clinical experience, ideally in geriatrics, acute / post-acute care, or rehab

Tech-savvy and EHR-proficient

Strong interpersonal and communication skills

Team-oriented with a collaborative mindset
