  • Role Overview
  • Assess patients in both inpatient and outpatient settings, crafting individualized treatment plans tailored to their unique needs. - Implement focused interventions aimed at enhancing mobility, strength, balance, and overall functional capabilities. - Collaborate closely with nurses, physicians, case managers, and fellow rehabilitation professionals to maximize functional outcomes. - Provide education to patients and their families about therapeutic goals, discharge processes, and home exercise regimens. - Maintain thorough documentation of evaluations, treatments, and progress according to facility protocols and regulatory standards. - Actively participate in team rounds and interdisciplinary meetings to ensure seamless and coordinated patient care.

  • Doctor of Physical Therapy (DPT) or equivalent in a recognized program. - Valid state licensure as a
