This position is eligible for commuter benefits for those who travel more than 20 miles one way from their residence to their work location.

What you bring to SBHG :

Masters degree in Social Work Psychology or Marriage Family and Child Counseling from an accredited college or university required

(4) years direct service experience with the mentally ill preferably adults preferred.

BBS Registered : APCC ACSW AMFT LPCC LCSW LMFT required

How you will make a difference :

Prepares a comprehensive written psychosocial evaluation and assessment on assigned clients in addition to an intake note daily progress notes summary notes (as may be required) and a discharge summary.

Responsible for assessing clients readiness for admission and coordination planning with treatment team the clients case managers and transferring facilities

Responsible for assessing clients readiness for discharge and coordination planning with treatment team; working with case managers to effect appropriate referrals for placement case management and aftercare services; coordinates benefits (Medi-Cal) with Business Office personnel; and maintains knowledge of community resources.

Provides individual group therapy to clients on a weekly basis and as needed based on the needs of the clients

Interprets facility policies and procedures to clients and family members.

Actively participates in treatment team meetings and is responsible for working with other disciplines to develop a coordinated treatment plan for assigned clients.

24 / 7 inpatient voluntary program

16-bed facility for adults (ages 18-59)

Rehabilitation / recovery services including substance use rehabilitation services

Pre-vocational or vocational counseling

Medication evaluation and support service
