DEPARTMENT : Renal Dialysis Unit
SCHEDULE : Flexible 8 Hour Day Shifts; Monday to Friday
This role will involve covering multiple outpatient dialysis centers.
Position Summary :
As a vital member of the multidisciplinary Nephrology team, you will assess, plan, implement, and evaluate the nutritional care of our patients, actively working to enhance their nutritional status.
Key Responsibilities :
- Collaborate with the healthcare team to ensure coordinated nutritional care for patients.
- Develop and tailor nutrition education materials to meet patient needs.
- Provide one-on-one counseling for individuals and families, adjusting advice based on personal and lifestyle factors.
- Conduct comprehensive nutritional assessments for chronic dialysis patients within one month of admission.
- Analyze clinical and laboratory data monthly, discussing findings with patients and documenting dietary counseling in their records.
- Complete dietary sections of care forms promptly after lab tests.
- Engage in interdisciplinary patient care conferences within thirty days of admission and biannually thereafter.
- Monitor Urea Reduction Ratio information and perform kinetic modeling, documenting adequacy results diligently.
- Take an active role in the Quality Assurance Committee to ensure high standards are met.
- Offer consultations for nursing home or acute renal failure admissions as necessary.
- Assist in onboarding new staff and provide educational in-services regularly.
- Exhibit organization and prioritization skills in managing tasks effectively.
- Maintain clear and respectful communication with all departments within the health system.
- Display the ability to function independently as well as collaboratively with the team.
Minimum Qualifications :
One year of clinical nutrition experience as a Registered Dietitian required.Bachelor's Degree from an accredited dietetics program required.Required Licensure / Certification Skills :
Registered Dietitian certification from the Commission on Dietetic Registration (CDR) is required.

PHYSICAL REQUIREMENTS :
Medium Work - Ability to exert up to 50 pounds occasionally, with frequent walking and standing.
