Active CA Dental License Dental Anesthesiology Specialty Program Certification BLS / CPR Certification PALS Certification (or willingness to obtain) Active DEA & NPI Malpractice insurance Fully mobile unit with crash cart and medications Enrollment (or willingness to enroll) with Denti-Cal, Delta Care, Delta Dental, and Liberty insurance plans
