GENERAL JOB DESCRIPTION
The Case Manager ensures the physical and mental well-being of clients They are responsible for
overseeing client's cases from intake to move out, providing the support necessary to achieve the
best outcomes.
DUTIES AND RESPONSIBILITIES
- Carries out case management activities as per the Part 900 Regulations and the Department of
Homeless Services (DHS) Case Management Guidelines for family shelters
Completes an assessment and fathers required documents (i.e. : birth certificates, socialsecurity cards, picture IDs, etc.)
Shows the Safe Sleep Video and carry out the appropriate documentation processMakes sure that clients sign the Code of Conduct and the facilities rules and regulationsObtains any possible proof of employment and assists with obtaining entitlementsMaintains clients' records by reviewing case notes; logging events and progressCommunicates clients' progress by conducting weekly interdisciplinary meetings andevaluations
Handles intakes of adults and ensures that all documentation is accurate and properly filedRecommends action plan and develops an Independent Living Programs (ILP) for clients that isreviewed and updated on a weekly basis
Collaborates with the Housing Specialist to create an exit strategy and target exit dateTasks family with seeking and accepting housingResponsible for meeting target goals set for each client by the Housing SpecialistConducts weekly unit inspectionsMaintains detailed records of each client in the form of progress notes through CARESProvides Director of Social Services (DSS) / Case Manager Supervisor with reports andsummaries of caseload for management review
Liaison for management, communicating clients' progress and any case concernsIn the event of an emergency, Essential Employees are required to report to work as scheduledDuties and responsibilities are subject to change based upon assessment of company needsSKILLS AND QUALIFICATIONS
Bachelor's Degree in Psychology, Social Work, Sociology, Mental Health, Nursing, HumanServices, or Education Services required.
Minimum of 2 years' experience working within the Social Services FieldMust have strong interpersonal skills and be able to communicate effectively (verbally and inwriting)
Must be organizedExperience with DHS Cares / Homeless population is a plusBilingual English / Spanish is a plus
