Join Gallagher Home Health Services as a Home Health Physical Therapist (PT) in the South Hills of Pittsburgh - Sign-On Bonus!

As a Physical Therapist with Gallagher Home Health, you will evaluate, plan, coordinate, and provide patient care based on the Physician's Plan of Treatment. You'll have the opportunity to make a direct impact on patients' lives while working from the comfort of their own homes.

Responsibilities

  • Provide comprehensive patient care through assessment and treatment planning in home health settings.
  • Utilize knowledge of physiology and anatomy to develop effective physical therapy interventions.
  • Collaborate with healthcare teams to create and implement care plans, ensuring continuity of care.
  • Educate patients and families on therapeutic exercises and discharge planning.
  • Maintain accurate medical records and documentation in accordance with regulations.

    • What We're Looking For :

  • Active Physical Therapist license in the State of Pennsylvania
  • At least 1 year of recent experience
  • Valid Pennsylvania Driver's License with no restrictions and a car with required insurance
  • Satisfactory Criminal Record Clearance
