Description :

The RN Case Manager provides essential care to patients in their homes.

You will get to know your patients and become an integral part of their care team.

The role offers broad independence while being part of a cohesive patient-care team.

As an RN Case Manager, you will screen, evaluate, re-evaluate, and treat patients.

You will coordinate and communicate treatment activities with interdisciplinary team members and patients.

Develop a care plan in collaboration with the patient's physician and family or primary caregivers upon admission.

Periodically review and recertify the care plan as appropriate.

Coordinate the patient's care plan through interdisciplinary communication and case conferences.

Advise other disciplines of discharges in a timely manner.

Provide ongoing assessment of the patient's clinical, functional, and emotional status.

Identify problems and progress toward desired goals.

Communicate significant changes to the physician and colleagues.

Document evaluations, treatment goals, and objectives properly.

Teach patients and caregivers self-care techniques as appropriate.

Provide medication, diet, and other instructions as ordered by the physician.

Utilize opportunities for health counseling with patients and caregivers.
