An experience-based sign on bonus : $5,000-$7,000 for LPNs and a $5,000-$10,000 for RNs (subject to applicable taxes) is available for this position! Please note that current UVM Health employees are excluded from eligibility for this bonus.

Post Acute atHelen Porter Rehabilitation & Nursing
The goal of the Post Acute Unit at UVMHN Helen Porter Rehabilitation & Nursing is to provide supportive, goal-directed care following surgery and / or hospitalization. Specially designed treatment programs provide patients with the preparation, support and education that enables them to return home as quickly as possible. Our goal is to help patients reach their highest possible level of independence. A partial list of conditions we work with includes strokes, neurological conditions, orthopedic care, respiratory conditions, and cardiac and post-trauma situations
Routine Therapy services are provided Monday through Friday. We also have the capability of providing therapy six days a week if that is what is required to maximize positive results. Nursing staff provide the necessary extension of therapies when therapy staff are not present to provide direct service. This may include walking, exercises and supervision of other activities set up by the therapy staff.
Charge Nurse Role :
Hours : 8am-4 : 30 pm
Days : Monday through Friday
Purpose :
The Charge Nurse works under the guidance of and in collaboration with the Post-Acute (PAC) Unit Manager. The Charge Nurse is responsible for assisting the PAC Manager in planning, implementing, and evaluation of nursing practice on the PAC unit. Under the guidance of the PAC Manager, the Charge Nurse is expected to provide clinical as well as administrative leadership for the unit. This includes creating an environment that provides patient care that is efficient, safe and of a high quality. The Charge Nurse is expected to work collaboratively with staff, physicians, and other administrative personnel for all units at Helen Porter Healthcare and Rehabilitation Center (HPHRC).
