State Licensed Clinical Liaison

A company is looking for a Clinical Liaison.

Key Responsibilities

Manage the immediate referral response process to meet performance goals

Conduct clinical assessments using electronic medical and client relation management systems

Evaluate prospective resident referrals based on clinical capabilities and communicate determinations effectively

Required Qualifications, Training, and Education

Active and in good standing Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of practice

Minimum of two years of prior experience in a similar role within a LTC / SNF / AL / MC setting

Intermediate to advanced computer skills, including experience with EMR or EHR

Ability to make independent decisions and handle confidential information

Strong leadership, organization, and time management skills
