The Physical Therapist assists in planning, organizing, developing, and directing Physical Therapy services in accordance with current applicable federal, state, and local standards, guidelines, and regulations, and as may be direction by Administrator, to assure the highest degree of quality resident care is always maintained.
Shift and Schedule : PRN as needed, averaging about 15 hours per week.
Job Functions :
- Provide physical therapy services to residents in accordance with established standards of practice, Company procedures, Therapy Department procedures and productivity standards.
- Adhere to Company procedures regarding documentation and billing of physical therapy services.
- Demonstrate sound judgment in the evaluation, planning, implementation, and follow-up of resident therapy programs.
- Conduct screening of residents at regular intervals to determine the need for intervention / treatment.
- Conduct timely screening of residents referred to physical therapy to determine need for intervention / treatment.
- Evaluate residents to obtain data necessary for treatment planning and implementation.
- Conduct specialized evaluations as indicated.
- Develop treatment plans by establishing short and long-term goals and methods to achieve identified goals.
- Interpret and communicate evaluation findings, treatment plan and recommendations to residents, families / responsible parties, and interdisciplinary team members.
- Perform all other duties as assigned and as required to effectively discharge the responsibilities of the position and are in the best interests of the organization.
- To complete compliant and medically necessary documentation from CMS expectations in NetHealth EMR systems
- Observes all safety rules and regulations.
- HIPAA Compliance – follows written policies and procedures to ensure confidentiality of protected health information.
- OIG Compliance – follows written policies and procedures to comply with the federal and state laws and regulations.
- Interacts with residents and coworkers in a friendly and respectful manner.
- Compliance with North Oaks Immunization requirements.
Education & Requirements :
Minimum of Bachelor of Science Degree in Physical Therapy from an accredited programMust have an active Physical Therapy license in state(s) of practice in good standingOngoing continuing education and professionalAssume responsibility for ongoing continuing education and professional development to maintain current licensure and certification.Physical Requirements / Working Conditions
Frequent standing, stooping, bending, stretching, squatting; may be exposed to blood and body fluids which may contain HIV and / or HBV; must be able to transport residents via wheelchair, gerichair or shower chair; pushing up to 150lbs., unassisted. Must be able to transfer, lift, turn, and position a resident weighing up to 50 lbs. unassisted or with the assistance of one for a resident weighing over 125 lbs. Must be able to stoop, bend, stretch, squat, and walk for up to 90% of the workday. May be subjected to offensive odors and combative behavior. Must be able to lift and carry up to 20 lbs. frequently, and 50 lbs occasionally.
