- Conduct H&Ps on new participants and perform event-based or periodic reassessments
- Develop medical treatment plans and provide medically necessary services across participant care settings
- Participate in intake and assessment evaluations with applicants and caregivers
- Discuss prognosis and care plans with participants and families to support shared decision-making
- Collaborate across nursing and interdisciplinary teams on treatment planning
- Actively contribute to team / family conferences and medical decision-making
- Educate patients and families on chronic conditions and health maintenance
- Supervise care plans developed by the interdisciplinary team
- Order and evaluate labs / diagnostics, document results systematically
- On-call responsibilities as required
