We are seeking a highly motivated and experienced Home Health Community Liaison (Marketer) to join our team in the Dallas - Ft. Worth Metroplex, TX area. The ideal candidate will have a proven track record of success in generating referrals and admissions for home health services.
Responsibilities
- Develop and maintain relationships with healthcare providers, hospitals, and other referral sources in the community.
- Identify and pursue new business opportunities to increase referrals and admissions.
- Educate referral sources on the benefits of home health services and our agency's capabilities.
- Collaborate with the clinical team to ensure a smooth transition of care for patients.
- Track and report on referral and admission activity to management.
