Descripción del trabajoPosition based in a utilization assessment department within a hospital setting Typical patient population includes individuals with complex or chronic health conditions Work schedule is 8 a.m. to 4 : 30 p.m. with weekday and rotating weekend shifts No on-call duties are required Floating may be required to facilities within a 60-mile radius Uses Epic for electronic charting; prior Epic experience preferred Conducts assessments and develops care coordination plans for patients with complex needs May require home visits and outreach in nontraditional settings to support patient care Collaborates across multidisciplinary teams to facilitate timely and appropriate care Assists patients in navigating healthcare systems and accessing community resources Uses evidence-based tools and quality measures to improve patient outcomes Must follow flu season vaccination requirements with medical exemptions only as allowed Local candidates within 60 miles are not eligible for travel positions Active CO or compact RN license required at time of consideration Completion of an accredited Registered Nursing Program required; BSN preferred At least 3 years of recent RN experience, preferably in case management, utilization review, managed care, population health, or chronic disease management American Heart Association (AHA) certifications required and must be current Experience with complex care coordination and independent decision-making preferred High Pay and Bonuses Medical, Dental and Vision Insurance with Day 1 Options 401(k) Plan Weekly Payroll Deposit Free Online CEUs Generous Housing Allowance Travel and Licensure Reimbursements Non-taxed Per Diem and Subsidy Traveler Rewards and Discounts
