About The Position

Job Title : RN Case Management / Discharge Planning

Experience : Minimum of 2 years in Acute Care Management with discharge planning experience, including at least 6 months at a large academic medical facility.

License : RN License Required

Certifications : BLS Required; RN Case Manager; CCM preferred but not required

Must-Have : EPIC Charting experience required

Description :

This position requires an RN Case Manager with experience in acute care management and discharge planning.

Candidates must have at least 2 years of relevant experience.

Experience should include a minimum of 6 months working in a large academic medical facility.

Candidates must possess EPIC charting experience.

The role involves managing medical discharges, with potential for some social discharges.

Candidates will cover all inpatient units throughout the facility.

The team currently consists of 15 Case Managers, with plans to expand.

The RN will work alongside a dedicated social work team.

Common patient types include those requiring complex placements.

The role includes consultation with respiratory and pharmacy as needed for discharge planning.

The department aims for efficient and effective discharge processes, ensuring readiness for patient placement.

Shifts are scheduled Monday through Friday, with weekend and holiday rotations as necessary.

There is a focus on collaboration with various departments to ensure comprehensive discharge planning.
