MedAdventures is looking for a dedicated Home Health Physical Therapist to join our Mifflin, PA team. In this role, you’ll provide personalized, one-on-one care in patients’ homes, helping them regain mobility, restore independence, and improve their overall quality of life. Recent home health experience with OASIS evaluations is required.

Your Responsibilities :

Conduct comprehensive home health evaluations using OASIS

Develop and implement individualized treatment plans

Deliver skilled PT interventions directly in patients’ homes

Document patient progress thoroughly and promptly

Collaborate with nursing, OT, and other team members to achieve optimal outcomes

Educate patients and caregivers on exercises, safety, and recovery strategies

Qualifications :

Active Pennsylvania PT license (or eligibility to obtain)

Minimum of one year of recent home health experience

Proficiency in OASIS documentation

Strong clinical reasoning, organizational, and communication skills

Commitment to providing compassionate, patient-centered care
