Job descriptionConduct comprehensive assessments, develop treatment plans, and implement evidence-based care practices. Monitor and evaluate patient progress, adjusting treatment plans as needed. Collaborate with interdisciplinary teams to ensure seamless patient care coordination. Chronic Wound Care Planning and Management : Educate patients and families on self-management strategies and lifestyle modifications. Communication and Collaboration : Collaborate with healthcare professionals, including physicians, nurses, and other specialists, to optimize patient care. Current and unrestricted Nurse Practitioner license in CO. Certification as a Nurse Practitioner with a specialization in wound care management is preferred. Minimum of 1 year of experience in wound care. Strong interpersonal and communication skills. Competitive compensation, per visit pay $125-160 depending on experience. Position Overview :
We are looking for a dynamic and autonomous Nurse Practitioner. The successful candidate will play a crucial role in providing direct patient care, developing chronic care plans, and collaborating with referring providers to optimize patient outcomes.
Responsibilities :
- Clinical Leadership :
Provide direct patient care through all available channels and products.
Develop personalized treatment plans for patients with wounds, focusing on long-term management and prevention of complications.
Establish and maintain effective communication channels with referring providers to ensure continuity of care.
