We are seeking a motivated and dedicated Independent Living Coordinator to join our team and support our mission.
Job Summary : The Independent Living Coordinator will be responsible for overseeing and coordinating services for individuals transitioning to independent living. This role involves developing personalized care plans, supervising caseloads, and ensuring that clients receive the support they need to thrive in their living environments. The ideal candidate will have experience in the field of developmental disabilities, excellent organizational skills, and a passion for helping others achieve their highest potential.
Key Responsibilities :
- Develop and implement individualized care plans for clients transitioning to independent living.
- Coordinate and supervise a team of personal attendants and support staff.
- Conduct regular assessments to monitor client progress and adjust care plans as needed.
- Facilitate communication between clients, families, staff, and other service providers.
- Provide training and support to staff to ensure high-quality service delivery.
- Organize and schedule quarterly, semi-annual, and annual meetings with service coordinators and families.
- Ensure compliance with all relevant regulations and standards.
- Maintain accurate and up-to-date documentation of client care and progress.
- Assist clients with daily living activities and promote independence.
- Address and resolve any issues or concerns related to client care.
