JOB SUMMARY :
Case Managers will provide individualized support, behavioral healthcare and non-healthcare services to clients who are overcoming and / or in the recovery stages of behavioral disorders such as mental illness, drug and substance abuse challenges.
ESSENTIAL JOB DUTIES & RESPONSIBILITIES :
- Provides individualized supports or care coordination of healthcare, behavioral healthcare, and non-healthcare services.
- Regularly administering screening questionnaires used to monitor Ssocial and environmental factors that influence health outcomes.
- Provides services to clients as related to social determinants of health including, but not limited to :
Housing / Neighborhood / Environmental needs
completion of clinical records, forms and reports

This position does not supervise any other position.
S KILLS & KNOWLEDGE :
TRAVEL :
Travel within the Cleveland area as needed for training and technical assistance. In addition, community travel needed to satisfy case management needs for clients.
SALARY RANGE :
Specialist II (Psychosocial Rehabilitation Specialist, 3 years exp) - Starting pay $20.00 per hour
Professional I, Behavioral Health Specialist; Case Management (Associates Degree) - Starting pay $21.00 per hour
Professional I, Behavioral Health Specialist; Case Management (Bachelors Degree) - Starting pay $22.00 per hour
WORK SCHEDULE : 40 hours per week
