Our therapy department partners closely with nursing, physicians, and case management to create individualized treatment plans that help residents achieve their highest level of independence and quality of life. Responsibilities * Evaluate residents and develop individualized occupational therapy treatment plans * Provide skilled occupational therapy interventions in accordance with physician orders and established care plans * Help patients improve performance in activities of daily living (ADLs), self-care tasks, mobility, cognition, and functional independence * Collaborate with Physical Therapists, Speech Therapists, nursing staff, physicians, and families to coordinate care * Complete timely and accurate documentation in compliance with regulatory and facility requirements * Participate in interdisciplinary team meetings, care conferences, and discharge planning * Educate patients, families, and caregivers regarding therapeutic techniques and home programs * Maintain compliance with all federal, state, and professional standards of practice Qualifications * Current California Occupational Therapist (OT) license in good standing * Graduate of an accredited Occupational Therapy program * Skilled nursing, post-acute, rehabilitation, or geriatric experience preferred * New graduates with strong clinical rotations are encouraged to apply * Experience with Casamba, Net Health, PointClickCare, or similar EMR systems is a plus * Strong communication, organizational, and teamwork skills * Passion for helping patients achieve their highest level of function and independence Ideal Candidate You are someone who: * Brings positivity and compassion to every patient interaction * Thrives in a team-oriented environment * Values clinical excellence and patient-centered care * Is committed to helping patients achieve meaningful outcomes * Enjoys working with the geriatric and post-acute population Join Our Rehab Team Today!
