Key Responsibilities
Manage a panel of high-risk patients, performing assessments and developing Palliative Plans of Care
Conduct consultative visits for advanced symptom management and complex goals of care
Participate in team activities including regular meetings and support during periods of rapid growth
Required Qualifications
An unrestricted advanced practice license in one or more of the specified states
Minimum of 5 years in advanced practice, with at least 3 years in Palliative care
ACHPN credential or willingness to obtain within the first six months of employment
