Vivo HealthStaff is working with a non-profit organization providing healthcare services to the geriatric population of the Bay Area.

We are looking for a Registered Nurse to work as a Care Management Supervisor. This role oversees and supervises care coordination and case management services, ensuring the integration of clinical services and adherence to program design. The supervisor ensures the completion of care plans, fosters a customer service-oriented environment, and trains staff in care coordination. They collaborate with the program manager and address any issues that arise. The supervisor also acts as a nursing consultant, provides education to individuals and families, and manages a caseload as needed. They are responsible for staff supervision, training, and performance evaluations, ensuring compliance with legal and regulatory requirements. The supervisor upholds nursing standards, and practices in accordance with HIPAA, and follows safety protocols. They actively participate in process improvement and maintain compliance with policies and procedures.
