Job Summary
The Clinical Care Partner MA is responsible for coordinating pre visit patient care following established standards and practices of population health management. Monitors disease registry for patient scheduled appointments, procedures, tests and referrals while communicating high risk patient needs to registered nurse care coordinator. Relies on guidelines to accomplish tasks. Relies on some experience to complete job. Works under close supervision.
Minimum Requirements
Experience - 3 years experience as a medical assistant
Education - Graduate of an accredited medical assistant program, or 2 years additional medical assistant experience in lieu of education.
Special Skills - Proficient in the English language, verbal and written communication skills, computer skills
Licensure – BLS Certification
