COMPANY OVERVIEW
Palm Beach Care Management (PBCM) is a private-pay aging life care practice with twelve years of history serving seniors, adults with chronic illness, and their families across Palm Beach County. PBCM provides individualized care management including assessment, care planning, coordination, advocacy, and family support. The practice is growing and building a structured operational model that delivers consistent, high-quality service at scale.

POSITION SUMMARY
This position is full-time, Monday- Friday with rotating on-call availability including evenings and weekends on a scheduled basis.

The Care Manager delivers high-quality, client-centered aging life care services in the field while maintaining same-day documentation, accurate billing support, and proactive communication with families, aides, physicians, and the supervisor. This is a field-based role that combines clinical judgment, advocacy, care coordination, and operational discipline. It is not a passive or flexible-schedule position - it requires organized, responsive professionals who are committed to follow-through, documentation accuracy, and the daily rhythms of a well-run care management practice.


Care management is a valuable and billable professional service. Care Managers at PBCM are expected to approach time tracking, billing entry, and documentation not as administrative burdens, but as core professional standards that reflect the value of the care they deliver. Candidates coming from salaried or non-billing clinical environments must be prepared to embrace this as a professional shift, not an adjustment.


This position performs the essential functions described below with or without reasonable accommodation.


CORE OUTCOMES
A Care Manager at PBCM consistently delivers:
  • Clients seen as scheduled with no unaccounted visits or gaps in service.
  • Daily Logs completed in Excel and MyJunna by end of business day on the day of each visit - without exception.
  • Billing entries accurate, complete, and submitted same-day.
  • Families updated weekly; providers and aides communicated with professionally and on time.
  • Changes in client condition, safety risks, and care-plan gaps identified and escalated promptly.
  • Schedules submitted to supervisor every Friday by noon, reviewed every Sunday evening.
ESSENTIAL FUNCTIONS
The following functions are essential to this position. Reasonable accommodation will be provided to enable qualified individuals with disabilities to perform these functions.
  1. Conduct client visits in homes, assisted living facilities (ALFs), skilled nursing facilities (SNFs), physician offices, hospitals, and other care settings as assigned; assess physical, cognitive, emotional, environmental, and care-plan needs during each visit.
  2. Complete the Daily Log in Excel and MyJunna visit notes with billing entries for each client by end of business day on the day of the visit. Check pill box, attach required photographs (pillbox, living space, vitals whiteboard, meals, and pertinent findings), and document all observations, concerns, and follow-up actions. If it is not documented, it did not happen.
  3. Prepare the upcoming week’s client schedule in MyJunna and Outlook by Friday at noon; recheck and adjust the schedule on Sunday evening; check in with the supervisor at the beginning and end of each workday.
  4. Communicate weekly with client families and as needed with physicians, aides, ALF staff, and other care team members; document all communications and follow-up actions in MyJunna.
  5. Identify changes in client condition, safety risks, behavioral shifts, or care-plan gaps and escalate to the supervisor promptly; do not wait for the next scheduled communication cycle.
  6. Coordinate services across providers - home health aides, physicians, specialists, pharmacies, and community resources - and ensure transitions between care settings are safe, documented, and followed up.
  7. Participate in care conferences, family meetings, and case reviews as assigned; prepare documentation in advance and submit notes afterward.
  8. Enter billable time accurately in MyJunna for each service rendered on the day of service. Flag any billing discrepancies to the supervisor the same day they are identified.
  9. Participate in shared after-hours and on-call coverage on a rotating schedule with fellow Care Managers. When taking scheduled time off, complete a formal client hand-off to the covering Care Manager including current care plans, pending follow-up items, and any active family concerns. No client is left uncovered.
  10. Maintain strict compliance with HIPAA, client confidentiality, company device and data policies, and all PBCM operational and professional standards.
  11. Complete all required documentation, reports, and supervisor check-ins accurately and on time. If it is not documented, it did not happen.
MARGINAL FUNCTIONS
The following duties are secondary and may be assigned based on business need.
  • Attend community events, referral meetings, or networking activities on behalf of PBCM.
  • Assist with intake documentation or onboarding for new clients when requested.
  • Support special projects, SOP documentation, or training content development as assigned.
MINIMUM QUALIFICATIONS
  • RN strongly preferred; qualified LPN with 10+ years of eldercare experience, MSW, or CMC credential considered based on demonstrated care management expertise.
  • Minimum of 3 years of experience in care management, eldercare, hospice, social work, or complex senior care coordination.
  • Demonstrated ability to assess older adults with cognitive and physical complexity, communicate with families across difficult situations, and coordinate services across multiple providers and settings.
  • Strong written communication skills; ability to produce accurate, clear, same-day clinical and billing documentation.
  • Comfort with care management software (MyJunna / Care Complete preferred), Microsoft Outlook, Google Sheets, and standard mobile applications.
  • Valid Florida driver’s license, reliable personal vehicle, current personal auto insurance, and ability to travel locally between multiple client locations each day throughout Palm Beach County.
  • Dedicated home office with reliable internet, secure computer, and ability to handle protected health information in full compliance with HIPAA.
PREFERRED QUALIFICATIONS
  • CMC (Certified Care Manager) credential or active pursuit of certification through NACCM.
  • Familiarity with Aging Life Care Professional® standards of practice and ALCA ethical guidelines.
  • Experience with MyJunna, Care Complete, or similar care management documentation and billing software.
  • Experience working with clients diagnosed with dementia, Alzheimer’s disease, or other cognitive impairment.
  • Familiarity with assisted living, memory care, and skilled nursing options in Palm Beach County.
KNOWLEDGE, SKILLS, AND ABILITIES
  • Clinical knowledge: assessment of older adults across physical, cognitive, emotional, environmental, and social domains; medication awareness; care-plan development and monitoring.
  • Documentation discipline: same-day, accurate, complete visit notes and billing entries - not end-of-week catch-ups.
  • Family systems: ability to work effectively with family members across complex relational dynamics, conflicting priorities, and high-stress situations.
  • Dementia and cognitive impairment: experience assessing and supporting clients with memory loss, behavioral symptoms, and safety risk.
  • Legal and incapacity planning awareness: working knowledge of the legal tools used by vulnerable adults and their families to manage incapacity, including power of attorney, healthcare surrogate designation, guardianship processes, and Medicaid planning considerations. Care Managers are expected to recognize when these instruments are missing, outdated, or need to be discussed, and to facilitate connection with appropriate legal and financial professionals.
  • Organization and follow-through: manage multiple clients and competing deadlines without letting anything fall through the cracks.
  • Professional boundaries: maintain warm, client-centered care while keeping appropriate boundaries with clients, families, and aides.
  • Communication: clear, timely written and verbal communication with supervisors, families, providers, and care team members.
  • Coachability: receive direction, feedback, and correction without defensiveness; adjust quickly and ask early when uncertain.
  • Technology: use smartphones, tablets, and documentation software consistently and accurately as a core part of the job.
PHYSICAL DEMANDS

WORK ENVIRONMENT
This is a field-based position. The Care Manager works primarily in client homes, residential care facilities, and healthcare settings, with administrative work performed from a dedicated home office. Exposure to the physical and emotional realities of aging, chronic illness, and end-of-life care is inherent to this role. A professional, composed, and empathetic approach is required in all settings at all times.
