Description : Position Purpose :
Join Mercy Care Atlanta as a Medical Case Manager , where you'll deliver patient-centered care and provide leadership to multidisciplinary teams. This advanced role focuses on supporting individuals living with HIV and / or HCV through care coordination, medical adherence, and resource connection to improve health outcomes.
What You Will Do :
- Coordinate care across Behavioral Health, Substance Use, Dental, Social Determinants of Health, and other specialties.
- Support adherence programs like STOPGap medication and long-acting injectables (e.g., Cabenuva).
- Develop personalized care plans and monitor medical progress, including viral load suppression and routine screenings.
- Refer patients to internal and external resources such as behavioral health, oral health services, or peer support.
- Maintain a caseload of up to 90 patients requiring intensive care coordination.
- Assist with quality improvement initiatives and Ryan White quarterly progress reports.
- Represent Mercy Care at community events and health fairs (3-4 annually).
