Facilitateteamconferencesweeklyandcoordinatealltreatmentplan modifications. Completecasemanagementaddendumsandallrequired documentation. Maintainknowledgeofregulations / standards,companypolicies / procedures,anddepartment operations. Review / analyzecasemanagementreports,includingKeyCareIndicators,andplanappropriate actions. Understandcommercialcontractlevels,exclusions,payorrequirements,andrecertification needs. Attend Acute Care Transfer (ACT) meetings to identify trends and collaboratively reduce ACTs. Meetwithpatient / familyperPatientArrivalandInitialVisitStandardwithin24hrs.of admission. Performassessmentofgoalsandcompletecasemanagementaddendumwithin48hoursof admission. Iflicensureisrequiredforone'sdisciplinewithinthestate,individualmustholdan active license. MustmeeteligibilityrequirementsforCCM®orACM™certificationuponentryintothis position OR within two years of entry into the position. CCM®orACM™certificationrequiredORmustbeobtainedwithintwoyearsofbeing placed in the Case Manager II position. For Nursing, must possess minimum of an Associate Degree in Nursing, RN licensure with BSN preferred. A diploma is acceptable only in those states whose minimum requirementforlicensureorcertificationisadiplomaratherthanan associate degree. Forallothereligiblelicensedorcertifiedhealthcareprofessionals,mustpossessa minimum of a bachelor's degree and graduate degree is preferred. 2yearsofrehabilitationexperience preferred.
