POSITION SUMMARY - RN Case Manager
POSITION DUTIES - Discharge planning, chart review and patient screening / Care Coordination / Barrier mitigation
MINIMUM REQUIRED QUALIFICATIONS - 1+ year acute hospital case management experience
RN license
LENGTH OF ASSIGNMENT - 13 weeks
SHIFT / HOURS PER WEEK - 8 : 30am-5pm / 40hrs per week
SYSTEMS - Meditech, Midas
START DATE - 12 / 30 / 2024
