About the Role :
The Violence Prevention Case Manager plays a critical role in providing case management services to youth impacted by violence and trauma. Under the direct supervision of the Director of Special Program Initiatives, this role is responsible for supporting youth and their families by providing essential services and resources. This position will serve as a liaison between clients, their families, schools, and community-based resources, ensuring seamless access to mental health services, social services, and educational support. This position will also include in-school programming to foster a positive, trauma-informed environment for youth within educational settings. The Case Manager must be accessible during both traditional and non-traditional hours, including nights and weekends, to ensure consistent support.
- Bachelor's degree in Social Work, Psychology, or a related field.
- Experience in case management or social services, particularly in a healthcare or community setting.
